5. The request must include your signature:
   Complete complaints require your physical or electronic signature or the signature of a representative authorized to act on your behalf. To satisfy this requirement, you (or your representative) may type your full legal name to act as your signature at the bottom of the complaint or on the Quizlet DMCA form.
